He needs a kick in the arse. Despite all the $h!t people throw at him these days, I never ever questioned his talent (unlike a lot of Bayern fans btw.). And he always performed for die Mannschaft at least. But he needs to change his attitude, he couldn't handle the hype after the WC, that's why he more or less stagnated ever since. He seems to have forgotten that you need to work hard, need to work on your weaknesses and try to improve every day.
He needs a good, but strict coach who can form him, who believes in him. And he needs to get out of Munich, the 'Schickeria' there (Munich high society, which is pretty damn superficial, annoying and plain retarded) doesn't help. Milan isn't the best destination in this respect though, I'd rather see him at Juve because of this. And Salihamidzic is already there, would make it easier for him to integrate. And the fickle Bayern fans don't do him good also - Bayern fans, unlike other fans, often are harsh to their own talents, while they tend to hype others.
Both Lahm's and Schweinsteiger's contracts end in 2009. I wouldn't be too surprised to see them both leave before that (especially Lahm).
