The first international game played was between the two teams to see who were world champions. Since then it follows on, you are world champions until you loose, it runs like a boxing world champion, you have to beat the champion to become the champion.
It should be the real world champion, not the unofficial world champion.

    you don't get nominated.

If for example, Uruguay has to play their next game against Ecuador, then that would automatically be the next World Champion match decider. If Ecuador win and then have a friendly against Western Samoa planned, and Western Samoa win that game, they are the next World Champion. and so on and so forth.
