I think and with no defensive midfielder on a player of kaka's quality there were always going to be problems. this is no reflection on gerrard, purely that rafa got his tactics wrong. it happens all over the world in matches every week. great players are outplayed and managers bring players on to change the tactics to try to change a game that is going against them
Milan's midfield was better than yours but no-one was picking up their markers (most notably Gerrard on Kaka). When Hamann came on, he did all the defensive work and allowed the others to play (similar to Makelele's role for Real Madrid).

Because Rafa is a little bit clueless? I think so. He's always been a fantastic defensive coach, but his Liverpool team's never really been very competent in attack.
